Create an Incident
Things to Consider
Prerequisites
Add the Incidents Tool to the Project Tools menu. See Add and Remove Project Tools.
Optional: If your company wants to configure the required fieldsets for data entry, complete the steps in Create New Configurable Fieldsets.
Steps
Navigate to the project's Incidents tool.
Click the Incidents tab.
Click Create.
Add the following information, as applicable:
Root Cause Analysis: Show / Hide StepsHazard. Select the hazard the person affected was involved with (e.g., Caught In).
Contributing Condition. Select the condition that contributed to the incident (e.g., Defective Equipment).
Contributing Behavior. Select the behavior that contributed to the incident (e.g., Distraction).
Work Activity. Select the specific work activity or task being performed when the incident occurred (e.g., Trenching).
Equipment. Select or enter the machinery, tools, or equipment involved in the incident (e.g., Forklift).
Root Cause Description. Enter a comprehensive description of the fundamental reason why the incident occurred.
Click Create.
Optional: Add a record, witness statement, or action to the incident after it has been created.
